Private pay session: $120 per 55 minute session
Insurance
I accept the following insurance:
- Blue Cross Blue Shield (All plans EXCEPT Blue Value and Blue Local)
- United Healthcare
- CBHA
- Medcost
- Medicare
- Aetna
As a courtesy, I will check your mental health benefits and coverage prior to your first appointment (i.e. – check if you have a deductible to meet, what your co-pay will be, etc). Although the information I receive is generally correct, the information I have access to is not always updated in real time by your insurance company. To ensure you receive the most accurate information, contact your insurance company directly.
I’d recommend asking these questions to your insurance provider to help determine your benefits:
- Does my health insurance plan include mental health benefits?
- Do I have a deductible? If so, what is it and have I met it yet?
- Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
- Do I need written approval from my primary care physician in order for services to be covered?
If I am not in network with your insurance plan, I will provide you with a statement to submit directly to your insurance company. Based on your out of network benefits, you may be reimbursed by your insurance for out of network costs.
Payment
I accept cash, check and all major credit cards (including FSA and HSA cards) as forms of payment. All payments are due at the time of session.
Cancellation Policy
Unlike traditional doctors offices, your scheduled appointment time is only reserved for you. If you are unable to attend a session, please contact me as soon as possible. Appointments canceled with less than 24 hours notice will be charged the full private pay rate of $120. Appointments canceled with less than 48 hours notice will be charged $50.
